LuluCare Credentialing Requirements Manual

1. Purpose and Scope
This Credentialing Requirements Manual (the "Manual") establishes the minimum documentation, verification, renewal, and eligibility standards for Healthcare Professionals who use the LuluCare marketplace. It applies to applicants, active Healthcare Professionals, LuluCare credentialing personnel, and authorized Facility representatives.
This Manual supplements the LuluCare Marketplace Terms of Service, Healthcare Professional Agreement, Background Check & Credential Verification Policy, Community Standards & Acceptable Use Policy, Privacy Policy, and Facility-specific requirements. It is an operational requirements document and does not replace any legal, licensing, regulatory, accreditation, or Facility obligation.
The requirements and processes described in this Manual are operational today. Third-party background screening runs as the final onboarding step, as described in Section 10.
2. Credentialing Principles
LuluCare credentialing is designed to support patient safety, legal compliance, marketplace integrity, and reliable Facility staffing. Credentialing decisions are based on the role, jurisdiction, Assignment, Facility requirements, document quality, primary-source information, and other relevant eligibility information.
Submission of a document does not guarantee approval. LuluCare may request clarification, replacement documents, primary-source verification, additional screening, or updated information at any time.
3. General Eligibility Requirements
Each Healthcare Professional must:
• be at least eighteen years old;
• provide accurate legal identity and contact information;
• provide a verified United States mobile number;
• provide an emergency contact;
• maintain an individual LuluCare Account;
• possess the license, certification, registration, education, training, and experience required for the role;
• satisfy applicable background-authorization, exclusion, sanction, health-documentation, and Facility requirements;
• maintain current and authentic Credentials;
• promptly report material changes affecting eligibility;
• complete required electronic agreements and attestations, including acceptance of the Terms of Service, Privacy Policy, E-Sign Consent, and Healthcare Professional Agreement;
• remain eligible with LuluCare's Payment Processor when payouts are required.
For nurse eligibility, LuluCare currently collects and verifies: (1) government-ID identity verification; (2) a professional license; (3) CPR/BLS certification; (4) tuberculosis test or chest X-ray documentation; (5) background-check authorization (consent); (6) a verified United States mobile number; (7) an emergency contact; and (8) acceptance of the Terms of Service, Privacy Policy, E-Sign Consent, and Healthcare Professional Agreement.
4. Identity Verification
Applicants may be required to submit a current government-issued driver's license, state identification card, passport, or other approved identification. The document must be legible, unexpired unless LuluCare expressly permits otherwise, and consistent with the applicant's Account information.
LuluCare may require a selfie, live identity check, identity-verification questions, device verification, or additional documentation. The person completing verification must be the Account holder. Account sharing, borrowed identification, altered documents, or impersonation are prohibited.
Government-issued identification may be viewed and downloaded by a Facility at which the Healthcare Professional is booked (confirmed) for a shift, as described in Sections 22 and 23.
5. Professional License Requirements
A Healthcare Professional must hold an active, valid, unrestricted license or certification appropriate for each role claimed on the Platform. LuluCare may verify the license directly with the issuing board or primary-source database.
Required license information may include:
• legal name;
• license type and number;
• issuing state or authority;
• original issue date;
• expiration date;
• current status;
• restrictions, probation, discipline, suspension, or revocation history.
A license listed as expired, inactive, suspended, revoked, surrendered, restricted beyond the applicable Assignment, or otherwise ineligible may result in denial or suspension of eligibility.
6. Role-Specific Baseline Requirements
CNA applicants generally must provide:
• active California Certified Nursing Assistant certification or other applicable state credential;
• current government-issued identification;
• current Basic Life Support or CPR certification if required by LuluCare or the Facility;
• tuberculosis test or chest X-ray documentation and other required health documents;
• background-screening authorization (consent), upon which third-party screening and eligibility review are initiated as the final onboarding step (see Section 10);
• any Facility-required training or acknowledgments.
LVN applicants generally must provide:
• active, applicable vocational nursing license;
• current government-issued identification;
• current Basic Life Support certification;
• tuberculosis test or chest X-ray documentation and other required health documents;
• background-screening authorization (consent), upon which third-party screening and eligibility review are initiated as the final onboarding step (see Section 10);
• role-specific competency, experience, or Facility requirements.
RN applicants generally must provide:
• active, applicable registered nursing license;
• current government-issued identification;
• current Basic Life Support certification;
• tuberculosis test or chest X-ray documentation and other required health documents;
• background-screening authorization (consent), upon which third-party screening and eligibility review are initiated as the final onboarding step (see Section 10);
• specialty certifications, experience, competencies, or Facility requirements when applicable.
These are baseline marketplace requirements. LuluCare or a Facility may require additional documentation for a particular role or Assignment, including the occupational-health requirements described in Section 9.
7. Basic Life Support and CPR
When required, Basic Life Support or CPR certification must be issued by an organization or training provider accepted by LuluCare or the applicable Facility. The certification must be current, verifiable, and appropriate for healthcare-provider use when required.
Online-only courses may be rejected when a hands-on skills component is required. LuluCare may request the front and back of a card, digital certificate, verification code, course record, or issuing-provider confirmation.
8. Tuberculosis Clearance
Tuberculosis screening or clearance is a current requirement for nurse eligibility and may include a skin test, blood test, chest X-ray documentation, symptom review, or other method accepted by applicable law and Facility policy.
The required testing interval and acceptable documentation may vary by Facility, jurisdiction, prior test result, and public-health guidance. A positive screening result may require additional medical clearance before eligibility can be granted.
Tuberculosis and other health documents may be viewed and downloaded by a Facility at which the Healthcare Professional is booked (confirmed) for a shift, as described in Sections 22 and 23.
9. Occupational Health, Physical Examination, and Vaccination Requirements
LuluCare handles occupational-health requirements as separate, component requirements rather than a single bundled clearance:
• Tuberculosis screening. Tuberculosis test or chest X-ray documentation is a standing requirement for nurse eligibility, as described in Section 8.
• Physical examination (Facility requirement). A Facility may require a current physical examination as a condition of working that Facility's shifts. Where a Facility enables this requirement, the Healthcare Professional must have a physical-examination record on file that LuluCare has reviewed and approved and that is current — not past a stated expiration date, or, where no expiration is stated, dated within the last twelve (12) months — before booking that Facility's shifts. A physical examination is not a general marketplace requirement; it applies only where a Facility requires it.
• Influenza vaccination and masking. An influenza (flu) vaccination record is optional. A Healthcare Professional who does not have a current, approved flu vaccination on file is flagged as "mask-required," and that status is shown to Facilities so that masking can be applied in accordance with the Facility's policy. Providing a current flu vaccination clears the mask-required flag.
• Other vaccination records. A Facility may also require additional vaccination or immunity records (for example, COVID-19 vaccination) as a condition of working that Facility's shifts.
A Facility's specific occupational-health requirements are shown to the Healthcare Professional, and LuluCare reviews and approves the corresponding records before the Professional may book that Facility's shifts.
Healthcare Professionals should submit only the requested occupational-health record and should not upload patient records or unrelated medical information. LuluCare does not guarantee that any particular record will satisfy every Facility's requirements.
10. Background Screening
Background screening is the final step of onboarding. As the last onboarding step, LuluCare presents a standalone disclosure and captures the applicant's background-check authorization (consent), and upon that authorization initiates screening through LuluCare's designated consumer reporting agency (Checkr). Marketplace eligibility depends on the results. Required disclosures and authorizations are presented separately, as a standalone document, where applicable.
Screening may include criminal history, identity verification, employment or education verification, motor-vehicle records when relevant, and other legally permitted checks. LuluCare conducts individualized review where required and may request additional context or documentation before making a final eligibility decision. Adverse-action and applicant-rights procedures are set out in the Background Check & Credential Verification Policy.
11. Exclusion, Sanction, and Disciplinary Screening
LuluCare may screen Healthcare Professionals against federal and state healthcare exclusion lists, licensing-board records, sanctions databases, abuse or misconduct registries, debarment lists, and other lawful sources.
A confirmed exclusion, disqualifying sanction, prohibited disciplinary status, or loss of legal eligibility may result in immediate denial, suspension, or termination. Healthcare Professionals must promptly report any new exclusion, sanction, or licensing action.
12. Work History and Experience
Applicants may be required to provide work history, education, training, specialty experience, references, or competency information. Information must be accurate and complete.
LuluCare may verify prior employment, dates, roles, references, education, or training. A Facility may require a minimum amount of recent experience for a particular Assignment. General marketplace approval does not guarantee eligibility for every Facility or specialty.
13. Facility-Specific Requirements
Facilities may require additional lawful documents or qualifications, including:
• orientation or policy acknowledgments;
• specialty certifications;
• recent experience requirements;
• skills checklists or competency assessments;
• drug screening;
• respirator fit testing;
• vaccination, immunity, or declination documentation;
• Facility-specific background or exclusion screening;
• dress, equipment, identification, or training requirements.
Facility-specific requirements must be displayed or otherwise communicated when reasonably possible before the Healthcare Professional accepts the Assignment. LuluCare may restrict access to a Facility until all applicable requirements are verified.
14. Document Submission Standards
All uploaded documents must be:
• complete and legible;
• authentic and unaltered;
• current where expiration applies;
• submitted under the correct document category;
• attributable to the Healthcare Professional;
• free from concealed, cropped-out, or misleading information;
• provided in an accepted file type and size.
Screenshots may be accepted when they clearly display the official source, Healthcare Professional's name, credential status, and relevant dates. LuluCare may require the original file or primary-source verification.
15. Name Differences and Corrections
The name on Credentials should reasonably match the Healthcare Professional's verified identity. A Healthcare Professional with a legal name change, prior surname, hyphenation difference, or other discrepancy may be required to provide supporting documentation.
Users must not alter a Credential to make names match. LuluCare may record an approved former or alternate name for verification purposes.
16. Credential Statuses
LuluCare's administrative verification workflow currently uses the following statuses for each submitted document:
• Pending — received but not fully reviewed;
• Verified — accepted based on available verification;
• Rejected — not accepted because it is invalid, incomplete, illegible, inauthentic, or does not satisfy requirements.
LuluCare may, now or in future releases, also apply additional descriptive statuses such as:
• Expiring — approaching its expiration date;
• Expired — no longer current;
• Needs Information — additional documentation or clarification is required;
• Under Review — subject to further investigation or primary-source verification.
A document status and overall marketplace eligibility are separate. A Healthcare Professional may have one verified document while remaining ineligible because another requirement is incomplete.
17. Expiration and Renewal
Healthcare Professionals are responsible for renewing Credentials before expiration and uploading replacement documents promptly. LuluCare may send reminders as a courtesy but does not guarantee delivery.
LuluCare may prevent a Healthcare Professional from applying for, accepting, or performing Assignments when a required Credential is expired or will expire before the Assignment is completed. Replacement documents may require reverification before eligibility is restored.
18. Ongoing Monitoring and Reverification
LuluCare may periodically or continuously recheck license status, exclusions, sanctions, expiration dates, identity, insurance, background information where authorized, and other eligibility factors.
Reverification may occur at scheduled intervals, before an Assignment, after a complaint or incident, when information conflicts, when a Facility requests it, or when LuluCare changes its requirements. Healthcare Professionals must cooperate with reasonable reverification requests.
19. Duty to Report Changes
Healthcare Professionals must promptly notify LuluCare of:
• license or certification expiration, restriction, suspension, surrender, or revocation;
• disciplinary action or regulatory investigation when disclosure is required;
• exclusion, sanction, debarment, or loss of government-program eligibility;
• criminal charges or convictions when disclosure is legally required;
• loss of required insurance or health documentation;
• a legal name or identity change;
• information showing that a previously submitted document was inaccurate;
• any condition that makes the Healthcare Professional unable to safely or lawfully perform an Assignment.
20. Credential Fraud and Misrepresentation
Prohibited conduct includes forged, altered, borrowed, purchased, fabricated, expired, or misleading Credentials; use of another person's health or training records; false work history; concealed discipline; manipulated screenshots; and attempts to interfere with primary-source verification.
LuluCare may immediately suspend an Account while suspected fraud is reviewed. Confirmed fraud may result in permanent removal, cancellation of Assignments, notification to affected Facilities, preservation of evidence, recovery of improper payments, and reports to licensing boards, screening providers, regulators, or law enforcement where permitted or required.
21. LuluCare Administrative Review
Credentialing personnel may approve, reject, request more information, escalate for review, or place a Credential under investigation. Decisions should be based on consistent requirements, reliable evidence, patient-safety considerations, and applicable law.
LuluCare's credentialing decision concerns Platform eligibility. It is not a licensing-board determination, employment decision by a Facility, guarantee of competence, or guarantee of future eligibility.
22. Facility Responsibility
Facilities remain responsible for determining whether a Healthcare Professional satisfies the Facility's legal, regulatory, accreditation, contractual, clinical, and operational requirements.
LuluCare may share verification status, credential type, qualification information, and Credential expiration dates as permitted by the Privacy Policy. When a Healthcare Professional is booked (confirmed) for a shift at a Facility, that Facility may view and download the credential and compliance documents the Healthcare Professional has provided to LuluCare — including professional licenses and certifications; health and compliance records (for example CPR/BLS, TB test, physical, and vaccination records); and government-issued identification (for example a driver's license or passport) — for that Facility's licensing, compliance, credentialing, and recordkeeping purposes. A Facility may access these documents only for Healthcare Professionals booked on one of that Facility's shifts; Facilities with which the Healthcare Professional has not booked a shift cannot access them. LuluCare records each time a Facility views or downloads these documents. These documents are not made public. Facilities must protect credential information and the documents they receive, must use them only for the purposes described above, and may not use them for unrelated purposes.
23. Privacy and Data Handling
Credential information will be handled according to the LuluCare Privacy Policy and applicable law. Access may be limited to LuluCare personnel, service providers, Facilities, advisors, insurers, regulators, and others with a legitimate and lawful need. Government-issued identification and health documents are accessible to LuluCare administrators and, as described in Section 22, to a Facility at which the Healthcare Professional is booked (confirmed) for a shift. LuluCare records each such Facility access.
Healthcare Professionals must not upload patient records, PHI, or patient-identifying clinical information through credentialing features.
24. Reconsideration and Corrections
A Healthcare Professional may request reconsideration of a credentialing decision by submitting official, reliable, and relevant documentation to support@trylulucare.com.
A request does not automatically restore eligibility or pause a safety-related restriction. LuluCare may reject repetitive, abusive, unsupported, or untimely requests, subject to applicable legal rights.
25. Changes to Requirements
LuluCare may update credentialing requirements to reflect changes in law, licensing rules, Facility requirements, patient-safety standards, insurers, screening providers, or marketplace operations.
Material changes may be communicated through the Platform or electronically. Healthcare Professionals may be required to satisfy updated requirements before continuing to accept Assignments.
